UPMC Western Maryland Center for Clinical Resources

The UPMC Western Maryland Center for Clinical Resources is a source of support for patients managing chronic medical conditions such as diabetes, heart failure, and lung disease, or taking anticoagulation medication. Our goal is to effectively co-manage at-risk patients who have a chronic disease in an outpatient setting to improve their health.

Pharmacists are available at the Center for Clinical Resources to help with the management of your anticoagulation medicine.

When you take anticoagulation medications (such as Coumadin) to slow down the blood-clotting process, it requires careful monitoring to make sure the dose is right. Some of the ways our team can help you if you take this medication include:

  • Educating you about the medication
  • Reviewing your other current medications for possible interactions
  • Monitoring blood tests to measure the blood’s clotting ability
  • Adjusting the dose when necessary
New patients are always welcome and your healthcare provider can make the referral for you.

The Lung Disease Clinic at the UPMC Western Maryland Center for Clinical Resources is staffed by respiratory therapists who provide you with one-on-one education and support to help you manage your lung disease. Any time you visit the Lung Disease Clinic, you are always welcome to bring a support person with you.

The Lung Disease Management Team

Our respiratory therapists work with your physician and serve as another set of “eyes and ears.” Nurse practitioners and pharmacists also are available in the Lung Disease Clinic to assist you with managing your medications. The team uses videos, printed materials, and hands-on instruction to help you understand:

  • Your respiratory system and your disease process
  • How your medications work and how to use them properly
  • The signs and symptoms of a flare up and how to manage it most effectively

The respiratory therapists can assist you with questions or issues you may have with your respiratory equipment, such as home oxygen, CPAP, nebulizers, inhalers, etc. They also can provide education and other related resources to help you quit smoking.

Once you are established in the Lung Disease Clinic, the therapists are available to answer your questions and can even help you with a flare-up, especially if your physician is not available.

If you have diabetes, the team at the UPMC Western Maryland Center for Clinical Resources can be your partner in keeping your diabetes under control and living life to the fullest. Our team is a knowledgeable source for education and information about diabetes self-management.

Whether you are newly diagnosed or seem to constantly struggle with your diabetes, the team will tailor a plan to meet your individual needs. Our team works closely with your primary care provider to promote your overall good health. They also can help you navigate the healthcare system and coordinate your care.

The team can show you how to monitor your blood sugar, teach you how to make dietary and lifestyle changes to control your diabetes, help manage your medication, and provide encouragement and motivation along the way.

Diabetes Resources

Living Well with Diabetes

A group education program, Living Well with Diabetes, covers the many aspects of diabetes self-management and glycemic control. It’s an enjoyable day filled with practical information and tools you can use daily to make managing your diabetes easier. The teaching team includes a certified diabetes care and education specialist, nurse, registered dietitians, pharmacists, and a Wellness Coach/Activity Instructor.

Morning Session

  • Understanding the diabetes process and treatment options
  • Monitoring blood glucose and using your results
  • Preventing, detecting and treating acute and chronic complications
  • Developing personal strategies to address personal issues or concerns
  • Incorporating nutritional management into your lifestyle
  • Developing personal strategies for a healthy lifestyle

Lunch

  • A healthy lunch will be provided

Afternoon Session

  • Incorporating physical activity into your lifestyle
  • Using medications safely and for the maximum therapeutic benefit

A family member or support person is welcome to attend with you. This program is approved by the American Diabetes Association and is offered each month. PCP referral is required.

Individual sessions may be offered if unable to attend group program due to barriers such as poor vision or hearing, learning disabilities, or physical limitations which make group environment difficult.

Please note: This program is covered by most insurance plans. When calling for pre-authorization with your insurer, please use these codes for Diabetes Self-Management Training: G0109 – Diabetes Outpatient Self-Management training services, group session (2 or more) per 30 minutes.

Medical Nutrition Therapy

important resource is medical nutrition therapy (MNT). Provided by a registered dietitian, it includes working on behavioral or lifestyle changes and individualized meal planning. PCP referral is required.

The Heart Failure Management team at the UPMC Western Maryland Center for Clinical Resources can help you to improve your quality of life by managing the symptoms of heart failure.

Services We Provide

Our team at the UPMC Western Maryland Center for Clinical Resources helps by:

  • Providing education and support
  • Monitoring symptoms of heart failure
  • Assisting with risk factor modification
  • Managing medications

Who Benefits from the Program?

The clinic is for individuals who have been diagnosed with heart failure or a “weak ” heart muscle. Weakness of the heart muscle often follows heart attacks or uncontrolled high blood pressure. Other terms used to describe this include systolic dysfunction, diastolic dysfunction, or reduced ejection fraction.

What are the Benefits?

  • Increased knowledge of your disease and how to manage it
  • Improved ability to carry out activities of daily living
  • Improved quality of life
  • Reduced number of trips to the emergency room and admissions to the hospital
  • Improved outlook and emotional stability

How Does the Clinic Work?

The Clinic is designed to provide you and your family with education about your condition and how to control symptoms of heart failure. Our goal is to reduce the number of visits to the Emergency Room, prevent hospitalizations, and help you live a full and productive life.

Your first visit will last about 90 minutes and includes a history and physical exam, weight and vital sign recording, a 6-minute walk test to determine activity tolerance, and completion of a survey to determine how heart failure symptoms are affecting your life.

After your first visit, most visits last about 30 minutes and include weight recording, review of medications, basic assessments, and education, which can be done individually and in a group setting.

In addition to clinic visits, you will also be encouraged to call us if you have any questions or have worsening of symptoms related to heart failure. The Clinic staff will also contact you to follow up on any changes in medications or in your symptoms.

Medication Therapy Management is a service provided by pharmacists at the Center for Clinical Resources that helps to make sure your medicines are working to improve your health. It is very useful if you take medicines for more than one medical problem.

Helping You Manage Your Medications

The pharmacist can help you manage your medications in many ways, including:

  • Explaining what each medicine is for and how to take each one
  • Making sure you are taking the right amount at the right times
  • Checking to see if any other medicines, over-the-counter drugs, supplements or herbs can interact with each other and be harmful
  • Answering any questions you may have

As part of your visit, you will work with the pharmacist to develop an action plan that explains how to use your medicines so you feel your best. You will also get a personalized list of all your medicines, include the drug name, dose, reason for taking, time of day to take it, and any special instructions.

When necessary, the pharmacist may contact your primary care provider or other healthcare providers with recommendations for adjusting your medications.

During your first visit, you should bring the following items:

  • Your prescription bottles
  • Any over-the-counter medicines you take
  • Any herbs or supplements that you use
  • Testing supplies that you use (such as a glucose meter)
  • Medication pillboxes
  • The most recent copies of blood pressure readings, glucose testing or lab work results
