WMHS Foundation Announces Ted A. Wolfe Foundation Partnership

logoThe Western Maryland Health System Foundation announces a partnership with the Ted A. Wolfe Foundation to provide funding for medical services and educational expenses for Allegany County residents in need of financial assistance.  The Ted A Wolfe Foundation was established by direction of the final will and testament of Mr. Wolfe and is administered by the WMHS Foundation.

Community organizations that provide services to Allegany County residents are encouraged to make requests for funding up to $20,000 based on well documented need.  The funds can be used to support existing programs or projects or as start-up funds for new initiatives.

Ted Wolfe was born and reared in Cumberland, graduated from Allegany High School, and went on to receive a bachelor’s degree from the University of Maryland and a master’s degree from American University.  He was a successful real estate investor in the Washington, D.C. area.  He passed away in 2005, and through his will, left a generous gift to the people of Allegany County.  As per his written wishes, his estate will be used exclusively for Allegany County residents. 

Applications for funding will be reviewed semi-annually, with the first round due by November 15.  Applications may be obtained on the WMHS website at www.wmhs.com, or by calling the Foundation office at 240-964-8100.  

Funding Proposal Guidelines - Ted A. Wolfe Foundation 

Administered by the Western Maryland Health System Foundation

Purpose:  The purpose of the Ted A. Wolfe Foundation is to provide funding for medical and educational expenses for those in need of financial assistance.  Funding is limited to Allegany County residents, as per written instructions in Mr. Wolfe’s final will and testament.

Amount of Funding:  A maximum of $20,000 annually will be awarded to successful applicants.  Applicants are encouraged to make requests based on well-documented need, up to the $20,000 level. 

Notification of Funding:  Successful applicants will be notified by December 31, 2010.

To apply for funding, please submit the following:

I. Organization - One page summary of your organization
• Name of your organization
• Address
• Telephone and fax numbers
• Name and title of primary contact person
• Email address of primary contact person
• Organization’s Tax ID number (if applicable)
• Mission Statement/Purpose of Organization

II. Funding Need and Program Impact - One to two page summary of the need
• Proposed Activity, Program or Need for Funding
• Target Audience (who, how many, by when)
• Expected Outcomes (Individual and Community)
• Amount requested, with explanation of need
• Any additional pertinent information

III. Attachments
• Organization’s current annual budget
• Budget for this particular project, including other sources of revenue or income
