REACH - Incentive Program for Healthy Lifestyles

REACH-Raising Employee Awareness for Continued Health, this program focuses on the factors that have the most impact on your health. Employees may qualify on two different levels.

Part A-is for all employees who want to learn about their health status and receive a $25 incentive bonus for participating in the Wellness Screenings in August.
Part B-is for employees covered under the WMHS medical plan and considered the primary member. Starting in January 2008, you can earn a reduction of up to $240 on your medical insurance if you meet the targeted eligibility criteria during the August screening. Note: you must maintain your enrollment and be the primary member in the WMHS employee medical plan to qualify for this incentive.

You qualify for Part A and $25 simply by participating in a brief wellness exam and blood screening in August to document your health status. You qualify for Part B by completing Part A and meeting at least four of the six criteria listed below.

  • Have a Body Mass Index (BMI of 27 or less or a 10% weight loss or 2 BMI loss between March and August 2007 (must be a documented loss)Note: Wellness Center will maintain documentation of baseline weights from the initial enrollment in the REACH program. BMI and weight loss can not be the same loss as from the previous REACH year.
  • Maintain targeted cholesterol level
    Total <= 200 mg/dl, and
    <= 2 risk factors, no coronary disease -- LDL <= 130
    >=3 risk factors, no coronary disease -- LDL <= 100
    Coronary event or equiv (diabetes, PVD) -- LDL <= 70
  • Maintain targeted blood pressure level
    No coronary history -- <= 140/90
    Coronary event or equiv (diabetes, PVD)--<=130/80
  • Have a blood glucose level of 110 mg/dl or less
  • Be tobacco free for at least three months prior to August 2007
  • Complete a Health Risk Appraisal HRA
    HRA forms will be made available to employees at the screenings listed below or can be obtained HERE. HRAs must be received by the Wellness Department by August 31,2007.
    If a medical condition prevents you from achieving any of these criteria, alternatives could be available. Please contact either the Wellness Center at (301)759-9355 or June Ward at (301)723-4472.

Deadline for enrolling in the REACH program and receipt of all documentation, such as lab results obtained outside of the REACH enrollment, as well as the HRAs, is the responsiblity of the employee and must be received by the Wellness Department by August 31, 2007
