2008 Medical and Dental Benefit Cost - (PDF File)	A printable cost sheet of the medical and dental cost per pay period based on the employee's authorized hours
MetLife Participating Dentist Network - (PDF File)	Listing of area dentist participating in plan
Dental Claim Form - (PDF File)	A printable form to submit for dental claim expenses
