Note: In an effort to manage prescription drug costs, if a medication is available generically, the plan will only pay for the lowest cost FDA approved generic at the Tier 1 (generic) copay level.  If a patient or physician desires the brand name equivalent of a medication that is available generically, or a specific higher cost generic product, then the patient will be charged the current Tier 4 copay, with a dispensing limit of 30-days supply.

Note: The WMHS Pharmacy Department monitors the Employee Prescription Program on an ongoing basis and will make changes in the formulary drugs listed and the applicable co pay amounts as required based on legal requirements, drug availability, and contract pricing availability.
