Medication Management

Medication Therapy Management is a service provided by pharmacists at the Center for Clinical Resources that helps to make sure your medicines are working to improve your health. It is very useful if you take medicines for more than one medical problem.

Helping You Manage Your Medications

The pharmacist can help you manage your medications in many ways, including:

  • Explaining what each medicine is for and how to take each one
  • Making sure you are taking the right amount at the right times
  • Checking to see if any other medicines, over-the-counter drugs, supplements or herbs can interact with each other and be harmful
  • Answering any questions you may have

As part of your visit, you will work with the pharmacist to develop an action plan that explains how to use your medicines so you feel your best. You will also get a personalized list of all your medicines, include the drug name, dose, reason for taking, time of day to take it, and any special instructions.

When necessary, the pharmacist may contact your primary care provider or other healthcare providers with recommendations for adjusting your medications.

During your first visit, you should bring the following items:

  • Your prescription bottles
  • Any over-the-counter medicines you take
  • Any herbs or supplements that you use
  • Testing supplies that you use (such as a glucose meter)
  • Medication pillboxes
  • The most recent copies of blood pressure readings, glucose testing or lab work results
